message). Contact page information is
used to help with your questions or suggestions.
We collect
personal information on our “Membership” page (e.g., name, degree, e-mail
address; institution / practice name, address, and phone number; fellow
training program information). Membership
page information is collected to help you become a member of IDSO. If you become a member of IDSO, your e-mail
address may be added to an IDSO member e-mail list, along with your name and
institution practice name, which we may use to contact you via e-mail in the
future.
When you
purchase an IDSO membership on idohio.org, during the checkout process, your
billing data (e.g., name, billing address, phone number, e-mail address) and
credit card payment data (e.g., card number, expiration date, and card security
code) are securely sent directly to the payment processor Authorize.net (https://www.authorize.net/about-us.html) in order to process your payment. Please refer to their privacy policy for
details (https://usa.visa.com/legal/privacy-policy.html).
We use
WooCommerce (https://woocommerce.com/) as our e-commerce platform to assist with purchasing an IDSO
membership. As part of the processing of
each order, we collect customer billing data (i.e., name, billing address,
phone number, e-mail address), and the last four numbers of the credit card
used and the expiration date. We do not
collect or store other credit card payment data on idohio.org. WooCommerce does
not receive or store customer billing data.

service purposes, contact page and membership page entries may be retained
indefinitely. Customer order information
is retained for as long as we are obliged to for administrative, legal, tax, or
security purposes.
What rights you have over your data
You can request
that we erase any personal data we hold about you. This does not include any
data we are obliged to keep for administrative, legal, tax, or security
purposes.
Where we send your data
The information
you submit on our membership page or contact us page is sent to a password
protected email address and is analyzed by a member of the IDSO staff.
How we protect your data
We use
encryption (SSL) on https://idohio.org/ to protect your data.
Changes to this privacy policy
We may update
this privacy policy periodically. When
the privacy policy is updated, the effective date listed at the top of this
document will be revised and the new privacy policy will be posted. Please review the privacy policy each time
